- Explore how soc/news-and-events/events/rcn-long-term-conditions-conference-170626/ial determinants of health contribute to health inequalities across major long-term conditions—including cancer, cardiovascular disease, gastroenterology, respiratory, and mental health—and identify nursing-led approaches to reduce disparities in both primary and secondary care settings.
- Showcase innovative, evidence-based practices that improve access, experience and outcomes for under-served populations living with long-term conditions, with a particular focus on integrated care models and cross-sector collaboration.
- Amplify national nursing voice and collaboration with other healthcare professionals that support equity-focused care delivery, enabling professionals to influence service design and patient engagement across the healthcare system.

The target audience includes nursing and healthcare professionals, particularly those in Bands 5–7, with an interest in long-term conditions (LTCs) and health inequalities. The event is open to both members and non-members.

Sarah Woolnough
Sarah is the Chief Executive of The King’s Fund, an independent charity working to improve people’s health. Since joining in late 2023, Sarah has overseen development of a new long term strategy, brand and operating model, all focused on boldly reimaging a health and care system that is compassionate, equitable and fit for the future, with people at its heart. The King’s Fund delivers its vision through research, analysis and insight, leadership and organisational development with health and care leaders, convening and events, and through partnership with others.
Previously, Sarah was the Chief Executive of Asthma + Lung UK, the national respiratory charity, having overseen the merger of the two leading lung health charities. Before that, Sarah spent more than 15 years at Cancer Research UK in a range of senior policy and executive board roles, and is a previous non-executive and trustee of Bliss, the spec/news-and-events/events/rcn-long-term-conditions-conference-170626/ial care baby charity, Action on Smoking and Health (ASH), the Assoc/news-and-events/events/rcn-long-term-conditions-conference-170626/iation of Medical Research Charities (AMRC), the National Cancer Research Institute (NCRI) and the Telstra Health UK Advisory Board.
Sarah has a background in policy and influencing, with degrees in government and an MSc in comparative politics. In 2011, she was seconded to the Department of Health to write the National Cancer Strategy alongside Professor Sir Mike Richards, the National Cancer Director. Sarah has chaired the Healthy Air Coalition, a collective of leading health and environmental charities campaigning for cleaner air and is a previous co-chair of the National Respiratory Delivery Board. Sarah previously worked in parl/news-and-events/events/rcn-long-term-conditions-conference-170626/iament, the civil service, and for membership organisation the Sport and Recreation All/news-and-events/events/rcn-long-term-conditions-conference-170626/iance.
Sarah is a current trustee of MQ: Transforming Mental Health.
Sarah is passionate about improving health and has led several national campaigns to deliver soc/news-and-events/events/rcn-long-term-conditions-conference-170626/ial change, including the plain packaging of tobacco and for bolder clean air laws. She has a particular interest in prevention, early d/news-and-events/events/rcn-long-term-conditions-conference-170626/iagnosis and community care, and the adoption and spread of innovation across the health and care system.
Adrienne Betteley
Adrienne is a nurse by background and has worked in a variety of settings including care homes and hospital but most of her nursing career was spent as a District Nursing Sister and Practice Educator working in Cheshire.
Adrienne then went on to do a number of different roles which were all focusing on End of Life Care in a variety of settings such as a Primary Care Trust, Cancer Networks and Strategic Health Authorities. She also undertook some national work on Advance Care Planning as part of the team leading on Preferred Priorities for Care in England.
Adrienne was also a trustee at her local hospice for 4 ½ years and a trustee of Care2Save for 2 years. She was also an elected Board member for the North West 国内精品伊人久久久久妇 Board for 2 years from 2006.
In 2011, Adrienne joined Macmillan Cancer Support and has worked there in a variety of roles, mostly focusing on Pall/news-and-events/events/rcn-long-term-conditions-conference-170626/iative and End of Life Care. Her current role commenced in January 2024 as Strategic Adviser for Cancer Care, which is UK wide and provides expertise and advice internally and externally. Adrienne previously represented Macmillan on several national groups in England such as the Ambitions Partnership of which she was elected as co-chair from January 2019 to march 2025. As part of Adrienne’s current role, she is a core member of the team leading Macmillan’s strategic work on cancer and other long term conditions. She also remains involved in a UK wide programme around Transformational Leadership in End of Life Care and Soc/news-and-events/events/rcn-long-term-conditions-conference-170626/ial Investment in End of Life Care. Alongside this Adrienne is the Co-Chair of Macmillan’s Network for Employees With a Lived Experience of Cancer having had two cancers herself.
Dr Matt Kearney
Matt was awarded the OBE in the New Years Honours List 2025 for services to health and the prevention of cardiovascular disease.
Dr Niloufar Nourishad
Dr Niloufar Nourishad is a Consultant Pharmacist in Adult Mental Health and the Director of the Pharmacy Education Programme at West London NHS Trust. She leads a psychopharmacology clinic within the Mental Health Integrated Network Teams (MINT) and contributes to the care of patients in high secure settings at Broadmoor Hospital. In parallel, she oversees research projects aimed at enhancing health literacy and optimising pharmacological treatment in the context of mental health disorders.
Luke Evans
国内精品伊人久久久久妇 Public Health Forum Chair
Assistant Delivery Manager in Health Equity – Guy’s and St Thomas’ NHS Foundation Trust
Luke qualified as a nurse in 2012 and has since completed his Professional Diploma in Tropical Nursing and his Masters in Global Public Health. He has worked across various sectors including; local government, international medical assistance, pharmaceuticals, health innovation networks and the NHS.
He’s been published in the British Journal of Nursing and Primary Care Nursing journals. He has worked on various public health init/news-and-events/events/rcn-long-term-conditions-conference-170626/iatives such as on the national team leading the Innovation for Health Inequalities Programme (InHIP) and performed a review of Health and Wellbeing roles within the NHS.
Luke is currently working in Health Equity, building the intelligence, infrastructure and interventions within his trust to deliver services so that they may provide equitable access, experiences and outcomes for their patients.
Luke joined the 国内精品伊人久久久久妇 Public Health Forum Committee in 2024 to raise awareness on the value of nurses working to improve systems and reducing inequalities.
Sharlene Hogan

国内精品伊人久久久久妇 Advanced Nurse Practitioner Forum Co-chair
Sharlene graduated from Brighton and Sussex University in 2011 with a masters in cardiology and qualified as a Non-Medical Prescriber in the same year. Her nursing career started in 1991 when she trained to become a registered general nurse at London South Bank University and qualified in 1994. Within the last five years, Sharlene has changed from the NHS to the Independent sector to join a pioneering group of Advanced Nursing Practitioners in a role that is new to the Clinical Research Organization (CRO) industry. Leveraging her clinical and research experience as well as extensive experience as an autonomous practitioner, she has applied these skills to the cardiovascular therapeutic arena. Mrs Hogan has been an ANP forum member for at least 5 years and is now the co-chair, in addition to this she has organized as well as hosted the ANP 国内精品伊人久久久久妇 Conferences and she has presented in Spain and Ireland about issues around Advanced Nursing Practice and she remains passionate about patient care, advancing the science of cardiovascular disease management and advancing nursing as a profession and career.
Naomi Watt
Naomi Watt is a Respiratory Spec/news-and-events/events/rcn-long-term-conditions-conference-170626/ialist Nurse with over 25 years of experience in primary and secondary care, public health and the third sector. She now works at Asthma + Lung UK, supporting clinic/news-and-events/events/rcn-long-term-conditions-conference-170626/ians in primary care and community settings to deliver the best possible care for people with lung conditions. An experienced educator, she also writes articles for healthcare journals. She is passionate about tackling health inequalities and making sure that everyone, no matter their background, has the chance to live well with better lung health.
Lena Johnson
While studying Biomedical Science, Lena worked part-time as a nursing support worker. Inspired by brill/news-and-events/events/rcn-long-term-conditions-conference-170626/iant nurses including her nurse parents, she completed a Post Graduate Diploma in Adult Nursing.
After qualifying, Lena worked in one of the largest medical acute admissions wards in central London gaining the knowledge and skills required to manage acutely unwell patients. She also completed her Advance Assessment training. During the Covid-19 pandemic, she managed 50 nursing staff on a respiratory ward, supporting Level 2 COVID-19 patients. In 2022, she joined the 国内精品伊人久久久久妇 to focus on safe and effective staffing, the 国内精品伊人久久久久妇 Nursing Workforce Standards, nurse-to-patient ratios and eradicating corridor care.
Lena prioritises patient safety and dignity, and staff education and wellbeing. She strongly beliefs that a knowledgeable workforce will deliver safe, effective, evidence-based, compassionate and high-quality care. Some of her proudest moments in nursing so far include receiving the High Sheriff of Greater London Award, Student Hero Award, Next Generation Nightingale Award, work done during the pandemic to support staff health, safety and wellbeing and the introduction of the 国内精品伊人久久久久妇 Standards Champions.
Lena enjoys gardening, dancing, cooking, volunteering and spending some quality time with family and friends.
Shauna Ann
Shauna is the Community Engagement Lead for Colostomy UK working to connect individuals, healthcare providers, community groups, and organisations to make a more stoma aware society. Shauna will be sharing real life experiences and research results to help provide practical insights to nursing staff to better understand the realities of life we a stoma.
Blerta Ilazi
Blerta Ilazi is a Health Inclusion Spec/news-and-events/events/rcn-long-term-conditions-conference-170626/ialist and a Queen Nurse whose professional portfolio encompasses clinical practice, education, and leadership.
In her role as an Advanced Nurse Practitioner, she provides dedicated care to patients from health inclusion groups.
In education, she works as a health inequalities educator with the South Yorkshire Primary Care Workforce & Training Hub and as a lecturer at the University of Sheffield’s School of Nursing and School of Medicine.
In her leadership role, she is the founder of ReSTORE program which supports refugee nurses to return to their carer by supporting them to join NHS workforce and pass the requirement exams as set by NMC.
A recognised national and international speaker, she advocates for inclusive and diverse workforce.
Kathryn Kemp
Dr. Kathryn Kemp is a General Practitioner with extensive experience working in primary care, spec/news-and-events/events/rcn-long-term-conditions-conference-170626/ialising in inclusion health. She is also a health inequality educator, committed to educating both primary care professionals and medical students on addressing disparities in healthcare.
Sarah Cross
Sarah Cross is an Adult Nurse lecturer at Cardiff university, working across undergraduate adult Nursing and post graduate Nursing and Allied Health Professionals. She spec/news-and-events/events/rcn-long-term-conditions-conference-170626/ialises in clinical assessment and management of cardiology and chronic disease and is Programme lead for the Independent prescribing programme
With 38yrs in Nursing and 20 years of this experience in Cardiology, Sarah has developed her cardiology background working in medical assessment, card/news-and-events/events/rcn-long-term-conditions-conference-170626/iac rehabilitation as a Clinical Nurse Spec/news-and-events/events/rcn-long-term-conditions-conference-170626/ialist, whilst developing an interest in Heart failure. On becoming a CNS in the Heart failure service she then went on to lead and develop, a community based heart failure service supporting intravenous / sub cutaneous diuretics delivery, in South East Wales. This service supported many patients with end stage and pall/news-and-events/events/rcn-long-term-conditions-conference-170626/iative heart failure.
Post pandemic this service evolved once more to become a responsive ambulatory heart failure service, providing a service of acute assessment and treatment avoiding hospitalisation to many, whilst continuing to offer optimisation clinic for the more stable HF patient population .
Mary Scott
Mary was born and bred in Sheffield.
Mary qualified as a State Registered Nurse in 1963.
Mary was d/news-and-events/events/rcn-long-term-conditions-conference-170626/iagnosed with COPD in 2002 which is controlled with medication and exercise.
Mary became involved with British Lung Foundation now Asthma + Lung UK . I am the secretary and chair of the Breathe Easy Support Group Sheffield.
Callum Metcalfe-O'Shea - Conference Chair

Callum has been working in primary care spec/news-and-events/events/rcn-long-term-conditions-conference-170626/ialising in d/news-and-events/events/rcn-long-term-conditions-conference-170626/iabetes as an Advanced Nurse Practitioner for the last 8 years, supporting the creation of a health professionals forum within his local area to improve d/news-and-events/events/rcn-long-term-conditions-conference-170626/iabetes education and training for nurses and other professionals. He has held various clinical and leadership roles, including clinical lead for d/news-and-events/events/rcn-long-term-conditions-conference-170626/iabetes at his local ICB. He still works clinically within general practice, is a clinical leadership advisor for the NHS Clinical Leaders Network and sits on various editor/news-and-events/events/rcn-long-term-conditions-conference-170626/ial boards including the Journal of General Practice Nursing. He has a strong passion for championing advanced practice in long-term condition management and now works at the 国内精品伊人久久久久妇 as the UK Professional Lead for Long-Term Conditions to enhance nursing practice and policy across the UK.
